By Mail:

• Request form by calling 1-800-Tax-Form
• complete IRS Form SS-4 five weeks prior to business opening
• Sign and mail to IRS service center for your state
• FEIN will be returned by mail in 4 weeks

By Telephone:

• Complete IRS Form SS-4 and call Tele-TIN number listed for your state
• Answer IRS agent questions and write FEIN agent give you in upper right hand corner of Form SS-4
• Mail the form to the nearest Tele-TIN unit within 24 hours or fax it to the IRS
• On your fax cover sheet include identification of your Form SS-4 as a Tele-TIN submission, the date, "attention IRS Tele-TIN office, Form SS-4", the FEIN you were given over the phone, your name and employer identification number and the number of pages faxed.
